Who Requires a DOT Physical?
You are required to complete and pass a DOT physical if you:
- Operate a motor vehicle for interstate commerce with a gross combination weight rating, gross vehicle weight rating, or gross combination weight of over 10,000 pounds.
- Operate a motor vehicle designed to carry more than 15 people.
- Are paid to operate a motor vehicle designed to carry more than eight people.
- Transport hazardous materials that require your vehicle to be placarded.
Components of the DOT Physical Exam

- Vital Signs Assessment: Measurement of blood pressure, heart rate, and respiratory rate.
- Vision Test: Ensuring at least 20/40 vision in each eye, with or without correction, and a field of vision of 70 degrees or more in each eye.
- Hearing Test: Ability to perceive a “forced whisper” at a distance of 5 feet, with or without hearing aids.
- Physical Examination: Evaluation of the heart, lungs, abdomen, neurological function, and overall physical fitness.
- Urinalysis: Testing for underlying health issues such as diabetes.
